Background Section 1886(b)(3)(B)(viii)(I) and (II) of the Act states that the applicable percentage increase for FY 2015 and each subsequent year shall be reduced by one-quarter of such applicable percentage increase (determined without regard to section 1886(b)(3)(B)(ix), (xi), or (xii) of the Act) for any subsection (d) hospital that does not submit data required to be submitted on measures specified by the Secretary in a form and manner, and at a time, specified by the Secretary
For Part B coverage, the injection must satisfy the following criteria: You must have a qualifying diagnosis (for example, pernicious anemia) Your provider must document the medical necessity The injection must be administered in an approved outpatient setting It is important to note that you may still be liable for your Part B deductible and 20% coinsurance, unless you have Medigap (supplemental insurance) to assist with out-of-pocket expenses
